DJP Update 11-1-2013 Some doctor friends in the news; LAGNIAPPE: Paracord bracelets

One of the joys of being involved over the years in organized medicine at the local and national level is the opportunity to meet dedicated doctors and staff of organizations throughout the USA (and beyond) and make new friends.

Here are examples of two friends in the news.  I know you can share others but I mention these individuals as illustrative of the dedication of physicians throughout the nation.

1- Dr. Robert S. Rigolosi received THE SPIRIT OF HEALING AWARD October 12, 2013, from Holy Name Medical Center for his medical achievements and commitment to outstanding patient care.  As most of you know, Bob is the current chair of the New Jersey Delegation the the AMA.

He received the award at THE FOUNDERS BALL held at a gala event at Pier Sixty in New York City.  Robin and I attended and it was truly gala.  Robin and I sat next to the wonderful Dr. Walter and Susan Kahn at the event.  Great fun.  Dr. Rigolosi has many achievements including creating a facility that is a model for hospital-based dialysis centers throughout the United States.  A brunch with Bob and Elaine, his professor wife and author, gave us the opportunity to visit further.

Bob’s medical accomplishments are well-known but here are two little known facts that I learned years ago when AMA president and visiting New Jersey.

a)  Bob was a champion boxer who made it to the finals in 1956 for the American boxing team for the Olympics.  He was offered an alternate spot but turned it down to finish his last semester in pre-med.  Because of his boxing experience and fluency in Italian, Bob was called to the 1960 Olympics in Rome and “asked to chaperone an 18-year-old Cassius Clay, who went on to win the gold medal and then changed his name to Muhammad Ali.”

b)  In addition, Bob was an actor in a number of Italian movies.  Love the posters of Bob in the movies!

Remember, every doctor you meet has a story outside of Medicine!

2-  Dr. Monica Wehby, pediatric neurosurgeon and member of the AMA Board of Trustees, is running for the United States Senate from Oregon!

If there is anyone who has not met Monica, that person is in for a real treat.  I had the privilege to testify with her before the Oregon Senate when I was AMA president advocating for medical liability reform in Oregon.  She is a dedicated neurosurgeon, smart, and articulate.  She does a great job testifying and handling questions using her experience as a neurosurgeon treating very ill children.  Check out her Website and Twitter posts.  See below.

LAGNIAPPE:  I always enjoy hearing of entrepreneurial adventures.  That is an important part of the American Spirit.  The individual who is the Webmaster for my Websites, Intrepid and On Leadership, decided to make paracord bracelets (made of parachute cord) and offer them for sale.  I noticed his “WhoDat” version on his wrist.  I ordered the same for myself and Robin.  Oldest daughter requested one too and she now has it.  These bracelets can be taken apart for 9 feet of very strong cord if needed for emergencies on camping trips etc.  Also, the WhoDat version has a whistle built in.

This is a very important issue!  Patient privacy is important in the patient-physician relationship.  Without that assurance, patients won’t tell physicians all necessary information to aid in diagnosis.  Sometimes important information can be embarrassing and patients won’t tell the doctor the info without assurance of confidentiality.  A Website that has patient info that is not secure puts the patient at risk for embarrassment,  identity theft, etc.  I hope AMA will discuss this critical issue of the EHR and the Portal Website at the Interim Meeting in November.  Without proof of patient privacy and medical record confidentiality, I believe the AMA is obligated to remove its support of PPACA.  AMA policy is opposed to the use of a unique patient identifier and now with the gov Website the patient’s social security number is listed!  Certainly that is a unique identifier.  Three other identifiers, without a single unique identifier, solves the correct identification without the unreasonable risk on a unique identifier such as Social Security Number via Web.

A bike that is awesome.  I brought my bike in for a repair and the bike show owner showed me the new Specialized Turbo and let me take it out for a ride.  It is as though a rocket is propelling the bike!  Read the article in Forbes in link below.  At the link you will find 2 videos.  Watch both.  They are short.  25+ miles per hour!  No, I did not buy one!

Update on 3 friends who served America during the Vietnam War

I do a lot of reading about WWII and have the advantage to visit frequently the National WWII Museum here in New Orleans.  But as a doctor who was in the U.S. Air Force during the Vietnam War, I always am interested in stories about friends who also served during during the years of the war in Vietnam.  Some stories, i.e., Paul Galanti, P.O.W., are in my leadership books and I give an update on Paul here.  Also, I now bring to your attention 2 other Americans, who are doctors, and who continue to help others by sharing knowledge for health and pleasure.

I had the privilege to get to know Dr. Carmona during my tour of duty on AMA Board and during AMA presidency.  I sat next to him at some dinners when he was keynote speaker.  He can mesmerized the audience when he speaks without notes or teleprompter and relates the American Dream as experienced by him.  On many occasions we ran into each other in the Atlanta Airport as we got a late dinner together, mine usually from Popeye’s and his a more healthy choice, between plane changes.

To shed some light on “what a bonsai is — how bonsai are developed and cared for,” there will be a presentation by Joseph Gutierrez, of the Potomac Bonsai Association. Visitors will have a close-up look “to learn a bit about how to judge a bonsai — to increase their appreciation of bonsai.” He will focus “on azaleas as bonsai, since azaleas do very well in this area, are readily available.”

Asked about the pots used for Bonsai, Guiterrez indicated that the pots must suit the tree and the style of the tree. “Some pots are considered to be quite “masculine,” as are the trees that are planted into them … e.g., heavy trunks; other trees are quite “feminine,” as some are very slender and graceful or flowering trees … and these are often planted into glazed or ornamented pots.”

DJP Update 9-20-2013 Thanks to Tulane Medical Alumni Assn (TMAA): DJP Outstanding Medical Alumnus 2013

Honored and humbled to be selected by Tulane Medical Alumni Association (TMAA) as Outstanding Medical Alumnus 2013.  The award will be given October 4.

Great appreciation to TMAA Board for selecting me and to the many outstanding medical mentors I have had during medical school, residency, and throughout my medical career.  They taught me and inspired me.

No doubt those mentors deserve this high honor more than I do.

And special thanks to family and friends who encouraged and helped me throughout my career in medicine.  We all know the sacrifices they make as we strive to help patients 24 hours a day.  I also reflect on a turning point in my life that many of you have heard me relate in speeches and in my leadership books.  That event was my decision to quit Tulane Medical School after the first anatomy test in the first semester.  I told my heroic policeman Dad that I did not think I was smart enough to be a doctor. This was his immediate response:

“Son, you are smart enough to be a doctor, but you must remember 3 things:  Do your homework, have courage, and don’t give up!  Do that and very little in life is impossible.”  I followed his advice, stayed in school, and that has made all the difference to me.  If only all kids who suffer hard times, sometimes without one or both parents, had such a loving mentor.  What a different world we would see.

For all the friends of Herb who could not attend the church services today, my eulogy for him is below.  And for all of us, attending a funeral or memorial service should cause us to reflect and tell our loved ones how much we appreciate and love them because we never know when we or they will depart for eternity.  That call or letter won’t be forgotten.

I told Herb’s family the sentiments of the out-of-town friends who wrote me this morning about Herb.  Thanks.

Herb left us on August 12, 2013, slipping away at home surrounded by his family; but his legacy lives on in his family and friends.  He was of great faith and left a legacy of being a man who sought after the wisdom of God, a husband who deeply cared for his wife, and a father and grandfather who was playful and giving.

We extend our sincere condolences to Herb’s family:  His wife, Beverly Nadine Burlison Ichinose; four children: Linda Williams, Lorna Reynolds, John Ichinose, Dr. Eugene Ichinose; his two stepsons, Jack and Rob Burlison, and 17 grandchildren.  His late wife, Elaine Okimoto Ichinose, and his parents, Samuro and Katsue Ichinose, preceded Herb in death.

Whenever I would think of Herb over the years, the word kindness immediately came to mind.  Always gracious, he was a gentleman and a gentle man. Whenever I called him for advice about the pathology in a case I was studying as a consultant to help defend a doctor, He always made the answer easy to understand and backed it up with outstanding references.  Herb remained the quintessential superb teacher who was patient and generous with his time.  If I wrote a book about a guru in a mystical Tibetan Shangri-La, Herb’s character would be the starring role.  Never did I hear anyone speak a word of dislike regarding Herb in the 50 plus years I knew him.  As one doctor stated today in an email, Herb combined brilliance and humility, a rare combination.

When I think of Herb, I also am reminded of words from Ralph Waldo Emerson writing about success:

To laugh often and love much;

to win the respect of intelligent persons

and the affection of children;

to earn the approbation of honest critics

to appreciate beauty;

to find the best in others;

to give of one’s self;

because you have lived,

this is to have succeeded.

I first met Herb when I was a medical student and he presented a case as a scholarly teacher at Tulane Medical School.  It was obvious to all that he was brilliant and kind.  I became friends with him and another professor friend of his, Dr. Glenn Kokame.  They enjoyed telling me that they met each other for the first time in the cafeteria at Tulane University as freshmen undergraduates.  Herb said none of the folks in the cafeteria looked like him but suddenly he saw a student behind him who did.  They introduced themselves and discovered they both were from Kauai, Hawaii!  That was the start of their wonderful friendship.

Herb became a full professor of pathology at Tulane Medical School and when he went into private practice he continued as a Clinical Professor of Pathology.  Always pursuing the unknown in the field of science he authored over 50 scientific papers and received many prestigious awards.  Despite the devastation he and Beverly suffered with Hurricane Katrina and his evacuation by a Coast Guard helicopter following Katrina, he never retired, continuing as a pathologist and embarking on the pursuit of anti-aging medicine.

When a loved one dies, the pain of loss can appear overwhelming.  But if we will reflect on the treasure trove of memories, the good times, the laughter, the moments of great joy, our pain will lessen and in time we will be left with comfort for his love and how he enriched our lives.

And let us remember what General George Patton, Jr. said:

It is foolish and wrong to mourn the men who died. Rather we should thank God that such men lived.

And I thank God that Herb lived and enriched our lives.  Today we celebrate his life.

I recently attended a Jesuit Retreat at Manresa, in Convent, Louisiana at the recommendation of my surgical partner, Dr. Jim Brown.  To sum up that retreat, the priest said:

“Remember, God loves you.”  Well, I am certain God loves Herb and the two of them are having wonderful philosophical discussions this very moment in heaven.  The poet Shelley spoke of “that sweet, strange mystery of what beyond these things may lie, and yet remain unseen.” But Faith, Hope, and Charity will keep our spirits joyful about Herb’s spirit’s new residence.

May we be so blest to have the love of God and all of family and friends!  Goodbye for now, dear Herb, We look forward to seeing you again in eternity.

DJP Update 6-16-2013 American Medical Association (AMA) Annual Meeting in progress + Lagniappe

Dateline:  Chicago

The policy of the AMA, policy that must be followed by the Board of Trustees, is made at the Annual and Interim Meetings of the AMA.  Resolutions are sent in by medical and specialty societies as well as individuals, and full debate and voting occurs.  There are reference committees to express viewpoints on the submitted resolutions.  Then the Reference Committee gives a summary and recommendations.  Finally, all resolutions go to the full House of Delegates (about 550 voting delegates) for more debate and a final vote.

There remains much debate between the meetings as to whether the AMA Board went beyond AMA policy in supporting PPACA, The Patient Protection and Affordable Care Act.  Some resolutions address removing support of PPACA.  The candidates for president-elect and plus those competing for the two available slots on the AMA board are facing questions such as “Would you advocate removing AMA support for PPACA?”  As the regulations and other disclosures of PPACA come to light, that is an important question.  Elections occur Tuesday morning.

As for my view of government intervention into Medicine, it remains the same as my first testimony in 1976 to Health Subcommittee of U.S. House Ways and Means.  The updated summary related to PPACA is in my Op-Ed 2010 Journal of the Louisiana State Medical Journal here:

Another issue that will generate more debate and eventually a vote at this meeting is the issue of whether AMA should continue two House of Delegates Meetings (now June and November) or reduce to one in June.  Some say new technology allows interaction with members and two meetings are no longer necessary.  New technology would include virtual reference committees via Internet.

Opponents of reduction to one meeting point out the need to have face to face debate at least twice a year. The two meetings give greater accountability of board and some members want more transparency.

I think two meetings increase AMA credibility and communication.  But regardless of what happens in this vote, I strongly urge AMA to move to an issue I have advocated for many years, prior to my board service and during my board service and AMA presidency.  AMA should have a closed circuit equivalent of C-Span.  All board meetings would be available live to members.  They could watch live or later at AMA Website via a recording by inserting membership number and password. Sensitive matters such as litigation or staffing could occur in closed non-broadcast mode.  If that was available, just like C-Span for Congress, AMA members could see how the board members debate, advocate an issue, and how they vote.  That gives accountability much more than what individuals say when running for office.  Perhaps someone could add this as an amendment to an appropriate pending resolution.  If AMA wants transparency and communication, this is a proven way to enhance it.  Leaders have courage and are decisive; they are accountable for their actions.  Transparency gives the “Verify” of “Trust and Verify.”  The financial cost of this would be minimal.

LAGNIAPPE:  Robin’s brother’s son, our nephew, just finished high school and visited us in the New Orleans area.  We had fun. Among our activities we went into the Honey Island Swamp and visited the National World War II Museum.  His grandfather fought as a U.S. Marine on Okinawa.  Powerful exhibit at WWII Museum on this battle.

1- Retreat: On March 7-10 I attended the famous Manresa House House of Retreats in Convent, Louisiana.  My surgical partner, Dr. Jim Brown, has been going for years and this was my first retreat there. The attendees do not speak during the visit.  There are talks by a visiting Jesuit in the mornings and evenings (30 minutes each talk) and church services daily but one does not have to attend anything.  You have a small private room for sleeping.  The meals are simple but excellent.  The grounds are full of oak trees and cypress trees.  A beautiful setting to mediate.  Regardless of one’s religious beliefs or even if one has no beliefs, being silent for 3 days and having no other responsibilities to interrupt meditation is worthwhile.  Some photos of the grounds at:  http://www.manresala.org/Facilities/Manresa-Grounds.aspx

5- I have blocked out 2 weeks in my schedule to remain at home and watch daily the outside repairs at our home.  No interviews, no speeches.  The patio, walkway to front door, sidewalks, and driveway had to be replaced because of sinkage.  We also added a chain wall on the South and North boundaries and on the West boundary at the end of the driveway.   A big job.  This time we found a real professional cement contractor and he is putting mini-pilings (sonotube with rebar) plus rebar steel and highway mesh at all sites.  In addition we are adding a gate for the driveway.  The gate posts are 8 inch square steel.  Gate design in progress for our West Indies style home.  Robin in charge of gate design after much research.  Amazing how a Bobcat picks those 500 pound posts up and puts them 4 feet into the ground.  That is an an impressive machine in the hands of a skilled operator!  Nothing like being present daily at job site when a myriad of questions arise.  Better to be there and state what you want rather than having them guess when a challenge occurs.  We are lining the driveway edges with brick and we did the patio in a 45 degree herringbone pattern of brick.  It is best to get involved in research before having anyone place bricks.  New brick?  Old brick? Mortar is different and important not to use modern mortar with old brick.

6- My restored greenhouse that suffered from Hurricane Katrina is now in great shape.  It is 17 feet long and 5 feet wide and can be entered from the back yard or from the house through French doors in our master dressing room by the master bedroom.

All is well in greenhouse.  I have a “Blueberry” grape plant from Napa that now has leaves on it.  I have a tomato plant and a rose bush with leaves and growing daily.  In addition, I have lots of different bromeliads plus an assortment of other plants such as strawberries, a Myer’s Asparagus fern, a poinsettia, an amaryllis, Tradescantia zebrina,  2 cacti, 2 blooming orchids, Swiss Chard, Kentucky Mint, Greek Oregano, Onion Chives, Rosemary, and Italian Parsley.

My secret to success:  the green house protects against freezing so plants such as the bromeliads do not die.  I have a homemade watering device that gives 20 minutes of drip water every 12 hours.  It is controlled by a battery powered timer and I use a hose that is filled with small holes for garden watering.  A most important technique is:  If a plant type dies in my greenhouse, I do not get that species again.  Sort of survival of the fittest for the environment I have in my green house.  This green house is a great source of peaceful meditation time and a wonderful opportunity to do macro photography.  It also serves as an intensive care unit for Robin’s indoor plants when they are near death.

7- Finally, our 50-year medical school graduation celebrations are in progress.  Orleans Parish Medical Society had a wonderful brunch for us.  My Tulane Medical School Class will meet in May.  I plan to interview my classmates regarding 50 years of practice.

Once again, I recommend you get on Twitter even if you don’t post tweets.  It is a terrific way to get the news on any topic you are interested in.  Just follow individuals who tweet about a subject you like.
For example, if there is a reporter you like, follow the reporter and you will get tweets giving links to breaking news long before you hear about it on TV or radio.  Reporters like to spread the news quickly!

2- Medical Meetings

A- I gave the keynote address at the Harris County Medical Society (HCMS) January 18, 2013 in Houston at the Petroleum Club.  Topic: Leadership.  Nice folks at this county medical society.  It is the largest county medical society in the world.

Russell W. H. Kridel, MD, FACS was installed as president of HCMS.  He gave a wonderful inauguration speech and I hope he publishes it.  In June 2012, Russ was elected chair-elect of the American Medical Association’s (AMA) Council on Science and Public Health.

Lagniappe was the invocation delivered by Dr. John Graham, a Tulane Medical School classmate who now is an episcopal priest.  Great seeing John.  He informed me he will be in New Orleans for the 50-year celebration of our medical school class in May.  Time flies when one is having fun!

B- This past weekend was the annual meeting of the Louisiana State Medical Society (LSMS).  It was fun seeing everyone and going to the past presidents’ dinner and catching up with these fine people.
Dr. Vincent (Van) A. Culotta, Jr. was installed as the 2013 president of the LSMS.  Van has done great work with the medical society and the Louisiana legislature.  Nice to see him get the recognition he deserves for his leadership.  He will be at AMA meetings as part of the LSMS Delegation.

Mardi Gras:  Tuesday, February 12, 2013.  Parades and parties already started!

The city is full of activity with the start of the Mardi Gras celebrations and the arrival of the teams and crowds for the Superbowl!  The Ravens are staying at the Hilton Riverside and the 49ers are at the Marriott on Canal Street.  Check out the story in the last tweet above.  If you are coming to town, don’t forget to check out my restaurant list at:

4- Media
There was an article today in The Wall Street Journal about employed physicians and unions.  http://online.wsj.com/article/SB10001424127887323375204578270401138739978.html?mod=googlenews_wsj

Fox News (Fox and Friends) called me today and asked me to be on the show tomorrow, Thursday, to comment on the article.  However, because of the activity with the Superbowl, no studios are available locally.  All booked up.  Interestingly also is the limo service Fox uses is completely booked up for transportation to and from studio.  I had planned to point out that the doctor responsibility is to the patient.  Collective bargaining can be of value for employed physicians who are not supervisors BUT the patient can never be held hostage.  I always make the point that my 6 commandments should not be broken:  SEC-C + 2.  Science, Ethics, Compassion, and Courage PLUS – Always do what is in the patient’s best interest, and – Do it with the patient’s informed consent.

Other items can be negotiated but never at the expense of the patient.  Thus I will not be on Fox in morning.  This points out to me the need to move faster with my plans to have a studio set up at home in my attic where I have an office and do my book writing.  With the new high-speed cable Wi-Fi I had installed and the studio microphone I am ordering, it becomes possible to broadcast worldwide using Skype.  I plan to get that technology completed in my attic and then can do media from home.  Innovation is exciting.  It is possible to take a picture of the background where you are seated and then have the computer substitute any other image you want instead as the background.  Thus my attic office can show in the background the Mississippi River bridge: The Crescent City Connection (CCC), formerly the Greater New Orleans Bridge or the Superdome.  Fun!

5- R.I.P. Paul Francis Naccari Jr., MD.  Robin and I attended the funeral today of Dr. Naccari.  He was one of the first physicians in Eastern New Orleans and on the board of Methodist Hospital in the East, the hospital destroyed by Hurricane Katrina.  Our time on Earth is short and physicians spend much of the time caring for patients.  One such physician has left us.  His medical practice partner, Dr. Terry Segura, gave a beautiful eulogy:  Eloquent, organized, and from the heart.
