This book should be in every medical school curriculum.  It is a cautionary tale for physicians and patients.  It deserves to be a best seller.  Uncovering the cause of a problem is a critical step in the cure.  Read and admire Dr. Constantian’s research, discovery, and solution offering hope for the future.
 
Lagniappe:  Did you know that JAMA and the New England Journal of Medicine no longer do book reviews?  In my opinion, that is a loss.  Encourage your state and specialty societies to review this book.  
Mark B. Constantian, MD, FACS, has practiced plastic surgery in Nashua, New Hampshire, since 1978 and has faculty appointments at the University of Wisconsin and the University of Virginia. He is the author of more than 100 professional journal articles and book chapters and two previous textbooks, including Rhinoplasty: Craft and Magic.”

Meticulously researched and illustrated with evocative vignettes, readers will enjoy this as much as a suspense thriller. Highly recommended for physicians and patients.

A plastic surgeon presents insights into patients’ motivations based on his practice and empirical research.
 
In this medical book, Constantian (Rhinoplasty, 2009) analyzes patients who have undergone multiple cosmetic surgeries and are unsatisfied with the results, finding that their reactions can often be connected to disturbing childhood experiences. The author combines anonymous anecdotes from his patients with peer-reviewed research into the lasting impacts of traumatic events in childhood to show that many cases are the result of patients’ reactions to family dysfunction or abuse. He argues that the pursuit of elective cosmetic surgery—rhinoplasties, or nose jobs, in Constantian’s practice—should be understood in that context. The deeply researched book (each chapter includes several pages of endnotes, and full credit is given to existing rubrics like the Mellody model) takes readers through existing literature on human psychology, including body dysmorphic disorder, an exploration of how behaviors related to body image can be a response to trauma, and the physiological effects of painful experiences. He concludes that patients can be best served by developing a sense of resilience and dealing with the underlying issues as opposed to going to surgeons who simply accede to requests to lengthen or shorten their nose tips by a few millimeters. The author urges physicians to understand the “intensity of emotion” that may be involved in a case and to acknowledge the connections between emotional state and physical health.
 
The writing here is strong, though certainly technical, and it is clear from the opening pages that the book is intended as a professional reference rather than casual reading material. The target audience is surgeons, and understanding that keeps the authoritative narrative tone from becoming overbearing (“Physicians see the effects of this neglect in patients who become childlike following surgery, or in Internet conversations where patients give medical advice to each other or pose questions that should be directed to their surgeons—or not even asked”). Although the patients who appear in the work’s many anecdotes may appear extreme (one’s “six-page letter read like the Unabomber Manifesto”), Constantian provides a level of detail and empathy that renders them entirely plausible, allowing readers to see how domineering parents, the lasting effects of physical abuse, or other childhood traumas can shape patients’ enduring unhealthy relationships with their bodies. The volume’s conclusions are based on solid science, and the author acknowledges socio-economic factors that may further shape patients’ responses. The occasional bits of humor (“If I were marooned with her for three days, she could probably turn me into Prufrock”) add a distinctive touch without detracting from the treatment of a serious subject. The concrete and actionable information provided gives readers useful takeaways, like comparisons of satisfaction rates between patients correcting real deformities and those having features that appear normal to a casual observer. There is some discussion of how patients can develop the resilience that seems to be the most effective way of managing these disorders, but the book remains focused on its readers, providing surgeons with guidance on improving their interactions with troublesome patients.
 
A solid, well-researched, and well-argued analysis of the behavior of plastic surgery patients.

 A- SEASON GREETINGS!

I hope everyone had a wonderful Thanksgiving celebration with family and loved ones.  We have much to be thankful for as Americans despite current day challenges.  As a veteran, I always give thanks to those I see in military uniform for their service and willingness to go into harm’s way.  Freedom and Liberty are not free.

Son Donald Jr. and wife Ana are in Metairie for Thanksgiving with our 4 Atlanta grandsons.  We visited the National WWII Museum in New Orleans as it continues to expand. Awesome museum.

1- Computer – My mid-2009 MacBook Pro 15“ and my 2010 MacBook Air are at the end of their lifespan.  Over the years I operated on them and put in new larger SDD hard drives and batteries but now aging problems are present beyond my repair abilities (non-functioning track pad etc.) and Apple won’t fix such old machines.  After much testing and evaluation for use at home and travel, I decided on the 2018 MacBook Pro 13” with touch bar, and the upgraded optional 16 GB RAM memory, faster optional processor and graphics, plus expanded 1 TB SSD hard drive. It is fast!  The Retina screen is great.

This computer may suffice for video production.  It is fast and has four Thunderbolt 3 connectors!

2- Clock – We recently acquired some cube shaped clocks, Brandstand BPECT Cubie Alarm Clock Charger for the nightstands in the bedrooms.  Each clock has 2 USB connectors plus two 3-prong plug in on top.  The clock runs on electricity but has one AA battery to power it if electricity is lost.  Works great.  Cost:  $58.95

D- LAGNIAPPE:  There comes a time in life when a person fully realizes that our time on earth is limited.  I also found that as one ages, the clock gives the sensation of moving quicker each day toward the end.  Thus Robin and I got our departure arrangements in order and started getting rid of items we collected over the years.  Often one realizes that a left behind item that one treasures is merely a burden for family to dispose.  Although I now realize my Mustang with 5.0 liter (302 cubic inch) Coyote engine will be accepted readily by all based on current family views.

For me, I always had trouble getting rid of books.  Books opened up adventures for me when I was at a boarding school for 6 years in grammar school.  We were mandated to stay in the study hall each evening for 2 hours.  That room contained the library.  I would finish homework early and the library books became my friends as I awaited release from the study hall.  This friendship allowed me to travel the world, spend 30 seconds over Tokyo during WWII, fight a crocodile with Bomba The Jungle Boy as he escaped the onrushing Marabunta Army Ants, and become a junior detective with The Hardy Boys.

Robin is the ultimate organizer and with her gentle persuasion, I came to the conclusion she was correct that we had to get rid of some possessions.  Recently we gave to the Jefferson Parish Friends of the Library over 250 books. (So far I have retained all of my research books for my upcoming novel!)

My elective seminar at Tulane University School of Medicine as clinical professor of surgery and clinical professor of medical jurisprudence is entitled:  HOW TO GIVE A MEMORABLE SPEECH.  It is part of my leadership seminar teaching.  The course has continually received great reviews from the students and it is now expanded to the professors.  I will be offering it on the road for organizations and for keynote speeches.  Stay tuned.

1- Thinking of all who were in the path of recent damaging hurricanes and the horrific fires in Napa and surrounding areas.  The psychological effects extend beyond the replacement of damaged property even when loved ones are not harmed.  For those who lost family and friends, our sincerest condolences.  In our area, Hurricane Katrina memories are vivid despite the passage of 12 years.

Distracted driving is an increasing hazard especially with texting and phone handling while driving!

According to the AAA Foundation for Traffic Safety, distracted driving is responsible for about 16 percent of all fatal motor vehicle crashes in the U.S. – which is about 5,000 deaths per year.  

The AAA Foundation also notes that, “Nearly half of all people who say they feel less safe than they did five years ago say distracted driving by other drivers fuels their concerns.”

Palmisano hopes to raise at least $25,000 for the campaign, which is aligned with a patient safety policy that MAG’s House of Delegates passed in 2016.   

Donors will be recognized on MAG’s website, MAG’s Facebook and Twitter accounts, and in the fourth quarter edition of the Journal of the Medical Association of Georgia

DJP Update 3-13-2017:  1- PPACA (ACA etc.) Repeal and Replace?  2- LAGNIAPPE: The Clock House and Bose QC 35, and don’t forget DJP News Enhanced Podcasts!

1.  Unless you have been cut off from communications since the November 2016 elections, you know there is a surprising change in the office of the President and in the Senate and House.  Pundits and polls for the most part got a failing grade.

 Regardless of your political views and your possible excitement or despair, one can be certain that health system reform is on the agenda in the immediate future.  It will be a test of leadership in Washington to see if reason and facts will prevail in the debate or if political rigidity and obstructionism continues based on political party lines.  This also is a chance for medical organizations to stand up and advocate long-standing policy rather than acting like the opportunists in the Vestibule of Hell in Dante’s Inferno (First part of Divine Comedy).

 I won’t dwell on this topic but it is an IMPORTANT issue for our patients and the profession of Medicine.  Time to enter the arena.

 1- THE CLOCK HOUSE

New Orleans is a fun place to visit because of the history, the people, food, and exciting traditions like MARDI GRAS and JAZZ FEST.  For those who like to stay in a private home rather than a hotel, Robin and I have a friend who has a place listed in AirBnB.  We visited the place. It is in Metairie and wonderful.  More details below:

 A brief summary:

“The Clock House” is a 1943 4-bedroom 2.5 bath home built in the “Grand Cottage” architectural style.  The main floor features a screened-in front porch, a gourmet kitchen, a breakfast area with a built-in banquette, and coved ceilings in the living and dining rooms.  Connecting bathrooms between the two bedrooms are on each side of the center hall on top floor. An arbor with a swing, table and bench grace the backyard where you can enjoy a garden view and bird watching at the feeder.  The home is one block from grocery, coffee shops, banks, and restaurants.  It is 5 miles to the French Quarter, 1.8 miles from the Streetcar line, and 1.6 miles to I-10 access.  The host lives in separate quarters on property.  Cereal, coffee, tea, snacks provided.  $275/night, (+ $150/reservation for housekeeping) for up to 8 guests  (babies & children count as guests.)  Fee is higher on special event weekends such as Mardi Gras, Jazz Fest and New Years.

 2- GADGETS:  Bose QC 35

 A) For trips to do keynote addresses and to attend board meetings, I try to protect my hearing in planes and block out the loud noise.  In the past, I used ear mufflers that are used on noisy construction sites.  But they are bulky.  I now have an ideal solution with additional benefits.  I recently purchased the new Bose QC 35.  The noise canceling is outstanding, the Bluetooth works well, and there is the added advantage of making phone calls with the two built-in microphones.
